We have made registration easy for you by offering your patient forms online. By offering you the option to fill out the forms in advance, you will be able to complete them at your own pace and in the comfort of your own home. Please make an appointment prior to filling out your new patient registration forms.
To begin the online registration process, simply select your registered office location from the menu and you will be directed to our secure DocuSign Patient forms. Please fill out every line on the documents to ensure you are seen in a timely manner and all your medical records are up to date. Once the forms are submitted, they are automatically sent to PHCA electronically.
